RADIOGRAPHY AND RADIATION PROTECTION: AN UPDATE

This talk will provide an update on the risks associated with ionising radiation, looking at the doses of dental radiographs and CBCT, and comparing these doses to the doses from common medical imaging procedure and other sources of radiation in our environment. Practical tips will be provided on how to optimise doses to patients. An update will also be provided on the IRMER 2017 and IRR 2017 legislation, with a focus on the latest, second edition “Guidance Notes for Dental Practitioners on the Safe Use of X-ray Equipment.”

Lee Feinberg, Consultant Dental and Maxillofacial Radiologist

Biography

Lee Feinberg is specialist in Dental and Maxillofacial Radiology and works as a Consultant Dental and Maxillofacial Radiologist at Kings College Hospital, London and at the Eastman Dental Hospital, University College London Hospitals. He is currently the Honorary Secretary of the British Society of Dental and Maxillofacial Radiology and sits on the council of the British Society of Head and Neck Imaging.

He completed his specialist training in Liverpool being trained by both Dental and Maxillofacial and ENT/Head and Neck Radiologists. He has clinical experience reporting Radiographs, CBCT, CT and MRI examinations. He performs specialist salivary gland imaging, diagnostic ultrasound and ultrasound-guided biopsies of the head and neck region.

Lee is passionate about education and holds a formal qualification in learning and teaching in higher education. He lectures regularly to postgraduate dental professionals. He has recently also taken up an Honorary Senior Clinical Lecturer role at Kings College London where he will be leading the undergraduate BDS teaching in Dental Radiology.

INFECTION PREVENTION IN DENTISTRY – IT’S A LOT MORE THAN JUST COVID-19

The safe delivery of dental care over the past three years has been strongly influenced by the publication of a range of standard operating procedures (SOPs) related to Covid-19.  The SOPs from different authorities are continually being updated and this presentation will highlight contemporary guidance for best practice during 2023.  Aspects for discussion will include vaccination, personal protective equipment (PPE) and surgery ventilation.  It is equally essential to ensure that decontamination processes are in place to prevent transmission of other viruses (herpes and hepatitis) and pathogenic bacteria (tuberculosis).  The impact of the pandemic on the prescribing of antimicrobial agents for dental infections will be outlined.  Finally, it is now expected that all clinicians should be able to recognise the potential for sepsis in a patient and therefore the clinical symptoms of this life-threatening condition will be presented.

Biography

Mike Lewis is Emeritus Professor at Cardiff University.  He has held a number of senior professional positions including, President of the British Society for Oral Medicine, President of the British Society for Oral & Dental Research, Dean of the Dental Faculty of the Royal College of Physicians & Surgeons of Glasgow, Dean of the School Dentistry at Cardiff University and Dental Member of the Department of Health specialist advisory committee on Antimicrobial Resistance and Hospital Acquired Infection. He has lectured extensively worldwide and is the author of textbooks on the subjects of Oral Medicine and Oral Microbiology.  Mike is a Registrant Member of the General Dental Council and was recently awarded the John Tomes Medal by the British Dental Association for outstanding service to the dental profession.

MOUTH AND OROFACIAL CANCER – WHERE ARE WE IN 2023?

It has been generally accepted that the incidence of mouth cancer within the UK population has consistently been increasing each year over the past three decades.   However, in 2022 it became apparent, as predicted by the dental profession, that the reduced access to clinical services during Covid-19 pandemic would result in a greater number of patients subsequently presenting with relatively large tumours and increased metastatic spread to the lymph nodes in the neck.   Both these features of mouth cancer are associated with a poor outcome.  In 2023, never has an effective dental examination, in particular a head and neck soft tissue examination, been so important for the public.  The updated Delivering Better Oral Health guidance for the UK contains sections related to mouth cancer to hopefully address these problems, in addition to presenting clinical actions required for cancer prevention going forward into 2023.  All dental professionals should also be able to recognise the different forms of skin cancer that may develop in the facial tissues or within the neck and be able to refer suspected malignancies to secondary healthcare services.

• Be able to accurately record benign and malignant mucosal abnormalities
• Have an understanding of the prevention and management of mucosal epithelial dysplasia

LEGAL AND ETHICAL ISSUES: TOP 10 RISK MANAGEMENT TIPS

Raj Rattan will present his top 10 risk management tips based on over 25 years of dento-legal and general dental practice experience. He will discuss each ‘tip’, illustrate it with practice examples, and present the dento-legal benefits of adopting the principles. He will cover:

• Basic principles of risk management
• Risk classification for general practice
• Managing operational risk
• Managing financial and business risk
• Communication risk to include social media
• Consent
• Classifying and Preventing failure
• Human error and how to prevent it

These themes will be presented as a 10—segment framework. Raj will also signpost delegates to further resources and reading materials for those who wish to study the subject further.

Raj Rattan MBE BDS MFGDP FFGDP Dip.MDE FICD FFLM

Raj Rattan’s portfolio career includes over 30 years’ clinical experience in general dental practice. He owned and operated a small practice group combining this with his involvement dental foundation training. Raj was a programme director for some 15 years before taking up the role of Strategic Associate Dean for foundation training at the London Deanery.  He has been an examiner for the FGDP and has been appointed an examiner for the faculty of forensic and legal medicine.

Raj has provided consultancy services to individuals and organisations including the NHS, Denplan, dental corporates and the Department of Health, where he has previously served as a Policy Adviser. He is the author/co-author of many textbooks on different aspects of general dental practice and has contributed articles to dental publications. He is well respected and popular international speaker. He worked a part-time dento-legal consultant at Dental Protection for many years before taking up his current role as Dental Director in 2016. 

DEVELOPING AN ORGANISATION SAFEGUARDING CULTURE

Safeguarding adults and children for all professionals remains high on the agenda for many reasons:

• The COVID -19 pandemic and lockdowns saw Domestic Abuse cases increase by 40% in some areas.
• The cost of living crisis is causing an increase in homelessness and Child Criminal Exploitation.
• Many people are arriving in the UK illegally having been victims of human trafficking and Modern Slavery.
• Requests to provide information for multi-agency reviews are widening in scope.
• The scrutiny of regulation and inspection is higher and more in focus than ever.

David Blain

Biography

Dave Blain is an independent safeguarding consultant with 42 years experience in the health sector prior to NHS retirement in 2021. A paramedic by clinical background, but the past 25 years working in child and adult safeguarding roles within various NHS services.  This included acute trusts, numerous ambulance services, Clinical Commissioning Groups, Primary Care and regional/national roles within NHS England/Improvement.

Dave was the national safeguarding advisor to the College of Paramedics from 2012 up until retirement, and has worked on numerous national safeguarding projects and documents with the Department of Health, Home Office, Health Education England and NHS England.

During Dave’s career he has been involved in over 100 safeguarding multi-agency reviews for children and adults.  Further relevant experience has also been gained from involvement in 34 Domestic Homicide/Suicide Reviews.

Dave holds a BSc (Hons) in Paramedic Science, a BA (Hons) in Teaching and Learning, and a Post Graduate Certificate in Education (PGCE).

THE DO’S AND DON’TS OF MEDICAL EMERGENCIES IN DENTAL PRACTICE

The adrenaline surge that comes with an unexpected medical emergency can make our brain work slightly differently! Having worked in emergencies for over 20 years Katie and Tracy, who will be presenting this talk, have observed common mistakes staff can make and will enlighten the dental community with known strategies to reduce mistakes. This presentation will cover the emergency equipment you are expected to keep in the dental practice, how to use it and how to keep it tamper free and ready for use. Every member of the dental team has an important role to play and can contribute to saving someone’s life.

Learning Objectives

Biography

Katie Baker is a senior NHS Resuscitation Officer at University Hospital Southampton. Previously an Emergency Department Sister, she now has 28 years in Critical Care attending in hospital medical emergencies as part of the in-hospital ‘Crash team’ which sees 50-60 emergencies and 10-15 cardiac arrests a month. Katie has an interest in baby & child resuscitation out of hospital and runs resuscitation updates & drills for Dental & Medical practices. She enjoys teaching and has taught around the UK and abroad. She is a registered Instructor and Course Director in Advanced Life Support with the Resuscitation Council (www.resus.org.uk) and a registered Advanced Paediatric Life Support Instructor (www.alsg.org).

Tracy Colmer is an NHS Resuscitation Officer at University Hospital Southampton NHS Foundation Trust. She is a Registered Instructor & Director in Advanced Life Support and runs the Generic Instructor Course. Tracey has over 30 years experience in the Acute Cardiac Unit and Resuscitation Services attending in hospital medical emergencies as part of the ‘Crash team’.
