INFECTION CONTROL AND PREVENTION IN THE COVID19 ERA

Outbreaks of emerging and re-emerging infections and more rarely pandemics such as HIV, swine flu and now SARS CoV-2 have always acted as drivers for change in clinical behaviour and technical innovation in dentistry. In this lecture we will examine the recent positive changes that have been made in infection control in dentistry that will make the profession more resilient in the future following the COVID 19 pandemic.

Aims:

To understand how the principles of dental infection control have been modified to make the dental surgery COVID 19 secure, as well as protect staff and patients from other infection risks.

Objectives

• Discuss why understanding how infections emerge can make healthcare more resilient in the future
• Discuss the current standard operating procedures for infection control and prevention
• Discuss best practice on dental surgery ventilation and implementing HTM03-01
• Review current occupational health measures in dentistry and the importance of occupational vaccines
• Understand the importance of maintaining antimicrobial stewardship and best prescribing principles

Dr Caroline Pankhurst BSc PhD BDS MSC MRCPath (Specialist in Oral Microbiology)

Caroline Pankhurst has published extensively on oral microbiology including two books on infection control most recently “Basic Guide to Prevention and Infection Control in Dentistry, 2nd edition”. She was formerly a Senior Lecture in Oral Microbiology at KCL Dental Institute and is now actively involved in postgraduate dental education and was a finalist in the e- Learning Awards 2012. She served on the SAC in Medical Microbiology, Royal College of Pathologists, Editorial Boards of “Primary Dental Care” & BMC Oral Health and a past President of the BDA Metropolitan Branch. She is a Trustee of the Fungal Infection Trust and the Affordable Health Initiative.

RADIOGRAPHY AND RADIATION PROTECTION: WHAT’S LURKING IN THE SHADOWS? – AN UPDATE ON RADIOGRAPHIC REPORTING AND CBCT

Radiology and radiography are used every day in general practice and are an important GDC core subject. This lecture is designed for the general practitioner to refresh and update their knowledge in this area.

The first part of the lecture describes some general principles of radiographic reporting and highlights some of the common difficulties in panoramic interpretation. The second part focuses on CBCT and will look at how these images are formed and when this modality should be considered to compliment plain images.

Some clinical examples will be presented at the end as an interactive discussion.

Learning Objectives

On completion of this lecture you should:

• Understand how to assess a panoramic image in a systemic way.
• Have refreshed your knowledge of anatomy on a panoramic image and be aware of how this may be affected by positioning variations.
• Understand how a CBCT image is formed and when it should be considered to compliment 2D imaging,
• Recognise some common abnormalities on panoramic images.

Paul Nixon graduated from University College London in 1986 and has been the consultant in dental and maxillofacial radiology in Liverpool for 20 years.  He has many years’ experience of undergraduate and post graduate teaching and has been the external examiner for the London University’s MSc in maxillofacial radiology.  He has also examined MFDS and MJDF for RCS Eng. and is currently an examiner for the Royal College of Radiologist’s diploma in dental and maxillofacial radiology.   He has written over 30 published papers and contributed to Tydesley’s Oral Medicine and Churchill’s Pocketbook of Clinical Dentistry.  He has recently been president of the British Society of Dental and Maxillofacial Radiology (2019-21).

WHERE ARE WE WITH REGARD TO MOUTH CANCER IN 2021?

The incidence of mouth cancer in the UK continues increase year on year with detection of the disease at Stage 1 or Sage 2 remaining low despite the introduction of Urgent Suspected Cancer referral pathways.  The reduced access to primary care caused by the COVID-19 pandemic has caused a significant reduction in referrals with an inevitable increase of patients presenting with advanced disease and poor outcome.  Recognition and appropriate referral of mouth cancer or oral potentially malignant disorders remains an essential aspect of dental care.  This lecture will present an interactive approach to the recognition, recording and referral (three Rs) of a spectrum of mucosal abnormalities, including squamous cell carcinoma, to ensure optimal patient management and the best use of specialist services in the present day healthcare environment.

• Have a contemporary understanding of mouth cancer and oral potentially malignant disorders.
• Gain an appreciation of the key clinical differences between benign and malignant mucosal disease.

Mike Lewis is Professor of Oral Medicine, at Cardiff University. He has held a number of senior professional positions including President of the British Society for Oral Medicine, President of the British Society for Oral & Dental Research, Dean of the Dental Faculty of the Royal College of Physicians & Surgeons of Glasgow and Dean of the School Dentistry, Cardiff University.  He has delivered more than 600 invited lectures and webinars worldwide. Mike is the co-author the best-selling textbooks, A Handbook of Oral Medicine and the BDJ Clinical Guide Oral Medicine in Primary Dental Care.

Sue Boynton is an Independent Dentolegal Consultant and The Complaint Expert who has already helped dentists and dental teams to resolve literally thousands of complaints. Sue not only uses her knowledge, experience and expertise to resolve complaints, she also excels at transforming negatives into positives.

She built her skills over many years working with an indemnity organisation where she was the knowledge expert for complaints, and also provided expert input into international complaints resolution procedures.

Sue is a hugely popular and effective expert who works 1:1, with teams, and with organisations where she transforms complaints, feedback, and reviews into successes. The fact that Sue is a dentist and a dentolegal consultant means that she can see all perspectives and she is known for being on your side. SAFEGUARDING CHILDREN VULNERABLE ADULTS, LESSON WE CAN LEARN FROM HIGH PROFILE CASES

Over the past decades there have been many child abuses cases that have prompted major overhauls to child protection procedures. A significant statement was made during the trial of Maire Kouao and Manning, that blame lay not only with the perpetrators but also with child protection authorities who were blindly incompetent. THE SYSTEMATIC APPROACH TO MEDICAL EMERGENCIES IN DENTAL PRACTICE

An ‘ABCDE’ approach to managing deteriorating patients has proved an effective method of recognising deteriorating patients in hospital. Now updated for community settings this presentation will take you through the recommended approach used by Emergency Healthcare Professionals and endorsed by the BDA.

Conditions such as Asthma, Diabetes and allergies are not uncommon. Recognising when your patient is deteriorating, taking steps to prevent them getting worse and how to recognise when they have become a medical emergency will be covered in this presentation. The value of teamwork between clinical and non-clinical staff, checking emergency equipment, communication and documentation will be emphasised and demonstrated.

Kate is a senior NHS Resuscitation Officer at University Hospital Southampton. Previously an Emergency Department Sister, she now has 28 years in Critical Care attending in hospital medical emergencies as part of the in-hospital ‘Crash team’ which sees 50-60 emergencies and 10-15 cardiac arrests a month. Kate has an interest in baby & child resuscitation out of hospital and runs resuscitation updates & drills for Dental & Medical practices. She enjoys teaching and has taught around the UK and abroad. She is a registered Instructor and Course Director in Advanced Life Support with the Resuscitation Council (www.resus.org.uk) and a registered Advanced Paediatric Life Support Instructor (www.alsg.org)
